GA Licensure by Endorsement

Process

Permanent License

  1. The process is similar for RNs and LPNs. Before submitting your license application, visit GAPS/Gemalto, Georgia's fingerprinting vendor, to register for your background check at least 48 hours in advance of submitting your application. Here are instructions for completing the background check for in-state applicants and out-of-state applicants.
  2. A valid photo ID will be required for fingerprinting, such as a State Issued Driver's License, US Passport, State Issued Identification Card, Government Issued Employee Identification Card, US Active Duty/Retiree/Reservist Military ID Card, or Tribal Identification Card.
  3.  Complete the licensure application form. You can find the form for RNs here and LPNs here. This form needs to be notarized and mailed in, online applications are not accepted.
  4.  After your application for licensure has been received and processed by the Board, you will receive an email from GAPS directing you to proceed to the fingerprinting site.
  5. You must proceed to a GAPS fingerprint site to have your fingerprints scanned within 90 days. If you fail to submit prints within 90 days, your registration will be canceled, a refund will be made to the method of payment you provided, and you will need to register again.
  6. A local law enforcement agency or private fingerprinting company may also provide fingerprint cards, but the applicant must ensure they are blue FBI fingerprint cards (FD-258).
  7. For out-of-state applicants: After your application for licensure has been received and processed by the Board, you will receive an email from GAPS directing you to mail your fingerprint cards. Please follow the emailed instructions and mail the fingerprint cards to:
  • Federal income tax return with a primary state of residence declaration
  1. Request verification of licensure via Nursys from your original state of licensure and the state where you are currently employed. If these states do not participate in Nursys, you will need to contact that state's BON and request verification be sent to the Georgia BON.
