Shift: Evening 3p-11, Night 11p-7a. Shift Length 8, 16
Guaranteed Hours; 48 hours per week
Type of Facility: 100 Residents Bed; 96 Semi-private beds, 4 private bed. 2 floors (1 LTC and 1 LTC-SNF). No locked units - Dementia and Alzheimer's patients wear a tracker that locks the door if they get close to it.
EMR: PCC in conjunction with paper charting.
Common diagnosis: LTC/SNF patient population: DM, COPD, Post Stroke, heart disease, fractures, dementia (Secure care tracker), Alzheimers, Rehab, chronic conditions, multiple comorbidities, and more.
Job Duties: Med passes, vitals, blood sugar checks and management, wound care, ostomy management, foley management, tube feeding administration, and assisting other team members.
Experience: Must have prior LTC/SNF experience.
Are RN's required to attend team conferences for acute inpatient rehab?: They are required to attend Daily Huddle.
Scheduler makes out the schedule 6 weeks in advance; travelers asked to stay flexible and help to fill gaps in permanent staffing.
Weekend Requirements: Every other weekend.
Holiday Requirements: May be asked to work holidays.
Call or Standby Requirement: None; Covered by perm staff.
Floating Requirements: Can float between either floor.
Dress Code: Any color scrubs, must look professional, no big earrings/jewelry, no fake nails; hair must be up; no face tattoos.
Department Specific Orientation: 3 shifts with a preceptor.
