Requirements

Experience

12 months of role experience is required from any number of months.

24 months of Home Health experience is required from last 12 months.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 manager/supervisor reference from any number of months in order to be considered for this role

Candidates must provide 1 charge/lead reference from any number of months in order to be considered for this role

License

Candidates must have a Virginia license or compact license

Locals accepted at same BR.

Coverage area is Hampton, Newport News, Seaford, Yorktown.

Shift info: Every third weekend

Skills include wounds, wounds vacs, Plurex, Central Lines, PICSS, lab draws, CP assessment, Be able to educate on Chronic Disease Management (COPD, CHF, DM, Pneumonia).

Keep RTO to a minimum, preferably no requests for calendar week off during assignment and no more than 7 days total. Any RTO must be in initial submission and if approved, also in confirmation document.
