Requirements

Experience

3 months of travel experience is preferred.

24 months of Physical Therapy experience is required.

3 months of experience with Epic is required.

References

Candidates must provide 1 manager/supervisor reference from within the past 24 months in order to be considered for this role

Candidates must provide 1 charge/lead reference from within the past 24 months in order to be considered for this role

Guaranteed Hours: 40

Outpatient Experience required.

Shift Monday-Friday 8:30a-4:45p.

Large adult outpatient clinic – 74% Medicare population.

Age group 12-100.

Patient diagnosis: balance/vestibular, neuro, Parkinson’s, neck/back, shoulders and knees.

All RTO or schedule requests must be on submission- HCF will not be approving RTO or schedule requests after submission. Please ensure HCP is flexible. Please upload all demographic information to the portal at time of submittal. Travelers are welcomed into the units and are expected to function as core staff does. Hard document deadline is Monday prior to start. No exceptions for pending docs.
