Requirements
Experience
24 months of role experience is required from any number of months.
12 months of Case Management experience is required from last 12 months.
3 months of experience with Epic is preferred from any number of months.
References
Candidates must provide 2 references from within the past 12 months in order to be considered for this role
License
Candidates must have a Georgia license or compact license

Additional Details:

Scheduling special requirements: 8-10 hour shifts

Rotating weekends.

Floating policy: Floating to needed coverage area

Years of Experience Required: 1 year acute care Case Management

Requested Skill Set: Interqual proficiency, Care Coordination, acute care case management experience

Must have RN previous Acute care Case Management expereince

Dress code: Professional casual

Shifts are Monday- Friday 8:00AM- 4:30 PM, Saturday, SundayAM 8:00 – 6pm

Cannot have an employment gap of more than 90 days (regardless of reason) in the past 12 months.
