24 months of Case Management experience is required from last 4 years.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Will they float to every department in the hospital or only specific units? will float to all units excluding surgery - first to float

What types of patients? the patient population is cardiac, surgical, oncology, critical care etc.

Candidates have recent experience in a California Dignity Health Hospital as a Care Coordinator.

Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered.
