Requirements

Experience

24 months of role experience is required from any number of months.

6 months of Physical Therapy experience is required from last 6 months.

3 months of experience with Cerner is preferred from any number of months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Additional Details:

Cannot have an employment gap of more than 90 days (regardless of reason) in the past 12 months.

Clinician to Patient Ratios : 1:1.

Type of equipment : typical equipment for rehab services. Treatment provided in patients room, hallway, outdoor areas, and in rehab gym.

Documentation system: Cerner and NetHealth.

Scheduling requirements: 40 hours per week, schedule needs to be reviewed during interview with rehab manager.

Floating policy: clinician will be asked to float between acute/swing, nursing home, and out-patient services as indicated.

Patient Experience requirements: Prefer clinician with at least 3 years of experience in medical setting.

Must have: Experience in hospital and/or nursing home setting.
