Requirements
Experience
12 months of role experience is required from any number of months.
12 months of Case Management experience is preferred from any number of months.
3 months of experience with Epic is required from any number of months.
References
Candidates must provide 3 manager/supervisor references from within the past 12 months in order to be considered for this role

Social Worker Case Manager - Required

Shift: may vary; will be put on a particular shift but needs someone willing to be flexible to move to a different shift as needed

Minimum Experience: 2 years acute care hospital – OR - 1 year acute + 1 year of Hospice, HH or other work with elderly

Required Certifications: MSW & LICSW or LSWAIC (only if they do not need supervision by MHS)

Skills required: Drug & Alcohol Screens (SBIRT), Mental Health Evals (SAFE-T), Capacity Evals, Behavior Plans, BH referrals, Provide or Assist with Tele Pysch Evals, Discharge planning, end of life conversations, Single Bed Certs/ITA process, difficult placements, prior experience in home health, hospice, SNF’s, Medicaid/Medicare, insurance authorizations.

Patient Types: Medical Surgical, ICU, Mother Baby, Labor and Delivery, ER coverage on weekend
