Requirements

Experience

12 months of role experience is required from any number of months.

12 months of Long Term Care experience is required from any number of months.

Experience with PointClickCare is preferred from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Minnesota license

Shift: D/N Rotate - EOW, Every other holiday

Do you allow self-scheduling or Block scheduling?: Block Scheduling available based on need

Type of Facility (Nursing home, skilled care, assisted living, memory care?): Nursing Home

Beds/Residents: 38 Residents

Ratios: Day/Evening shift ratio 1 nurse, 2-3 CNAs and 20 residents, NOC shift ratio 1 nurse, 2-3 CNAs and 40 residents

Common diagnosis / Types of patients: COPD, dementia, Parkinsons, MS, CHF

Experience or 'Must have' skills: pass meds, supervisory skills, previous nursing home experience a plus

Candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past year.

RTO needed upfront - Additional RTO will not be approved, including after submissions.
