Requirements

Experience

12 months of role experience is required from any number of months.

3 months of Home Health experience is required from last 3 years.

3 months of experience with Oasis is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

Candidates must have a Texas license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

Shift: Monday - Friday

Coverage area: Energy Corridor, Spring Branch, Spring Valley, Memorial, Katy

May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address

LPN productivity expectation for 40 hrs. = 32-36 points/week

Must have recent HH exp. 2023-24

Home Care Home Base experience required

Client may consider without HCHB exp if CLN completes online training class prior to start.

Any overtime must be pre-approved and requires written approval from Administrator
