Requirements

Experience

12 months of travel experience is required from any number of months.

12 months of Case Management experience is required from any number of months.

Experience with Epic is required from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Must Be/Have: Experienced CM • a RN • Discharge planning assessment exp • Excellent UR • Psych social assessment skills • Acute hospital setting experience. Requires doing InterQuals & screening transfers for clinical appropriateness.
