Requirements
Experience
12 months of role experience is required from last 3 years.
12 months of Hospice experience is required from last 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Ohio license or compact license

Specialty: RN Case Manager - Hospice
Schedule: Hospice On-Call RN Case Manager. Friday at 5pm to Monday at 8am weekly.
