Requirements

Experience

24 months of role experience is required from any number of months.

12 months of Home Health experience is required from last 4 years.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a license

Patient Types: Home health clients - surgeries, falls, etc.

Home Health and Hospice patients

Dress Code: CSH color scrub or Business Casual

Candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past 12 months.

RTO needed upfront - Additional RTO will not be approved, including after submissions.
