Requirements
Experience
24 months of role experience is required.
24 months of Transplant Unit experience is required.
3 months of experience with Epic is preferred.
References
Candidates must provide 2 manager/supervisor references from within the past 24 months in order to be considered for this role
License
Candidates must have a Maryland license or compact license

Additional Details:

Inpatient Nurse Transplant Coordinator: assistance in transitioning the patients to home for all Solid Organ Transplant patients, lung, heart, liver and kidney transplant patients.

Responsible for the primary education sessions, diabetic teaching, medication teaching, prescriptions, medication lists, discharge planning.

Required: 2 years current strong Med Surg experience and Transplant experience.

Preferred: Experience with kidney transplant coordination

Assumes responsibility for on call activities that include organizing transplants and triaging patient problems.

Shift: Mon-Fri

Help to coordinate care and clinically manage post-transplant recipients. This includes prescription refills, clinics, triaging calls, medication reconciliation, reviewing charts, collaboration with other transplant disciplines like pharmacy, social work and nutrition.
