Requirements

Experience

12 months of role experience is required from any number of months.

12 months of Home Health experience is required from last 3 years.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a license

Additional Information

Additional Details:

1 year of experience must be within a Skilled Nursing Facility.

Guaranteed Hours: 40

Cases Per Day: Up to 5 in single family homes, apartments or assisted living facilities about 30 to 40 mi radius.

A majority of the patients will be right in the Fargo Moorhead area

Will need to supervise 1 PTA

Will train new graduates

Completion of an accredited physical therapy program required.

Must be able to effectively communicate in English, both orally and in writing.
