Requirements

Experience

12 months of role experience is required from any number of months.

10 months of Home Health experience is required from last 3 months.

3 months of experience with Oasis is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

Candidates must have a Washington license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

-If you have a multistate/compact nursing license, you will be required to complete a one time 6 hour suicide prevention training according to the WA state regulations. (Nursing positions only).

** All clinicians will be required to start after 5/5. Please ensure you are setting this expectation with clinician prior to submitting**

RTO (Requested Time Off) must be submitted at the time of application.

Start Date: ASAP , Compliance takes about 2 weeks.

Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)

Weekend Required: see on call requirement.

Is On-Call Required?: Every 4th weekend and a weekday on-call after hours 2-3 x's per month.

Case Load: Geriatric.

Traveler is expected to have their own transportation.

Timely communication and documentation required.

Documentation tools will be provided, and must be returned.

Attire / Scrub Color: Any color scrubs.

Charting: HomeCare HomeBase (Pref- online course required prior to assignment) /Oasis (Required).
