Additional Details:

A minimum of 1 year acute care experience is required to work in acute care facilities. The following are considered acute care facilities: Hospital (General Acute Care as well as Psychiatric, Specialized and Rehabilitation Hospitals, and Long Term Acute Care or LTAC), and Ambulatory Care Facilities.

Guaranteed Hours: 40

Weekend Shift - Friday-Monday

Every other weekend

Orientation is weekly, on Mondays.

Certification: CCM/ACM preferred

Patient Population e.g. “geriatric, adult, peds, etc.”: geriatric, adult, and adolescent, women, and infant

Must Have Skills/Experience: Utilization Management, Utilization Review (InterQual), Discharge Planning, insurance, acute hospital case management experience, recent traveler dyad case management experience

Common Diagnosis/Treatment: CHF, Sepsis, COPD, Pneumonia, UTI, CVA, Acute Renal Failure
