Requirements

Experience

24 months of role experience is required.

36 months of Home Health experience is required.

Experience with Oasis is preferred.

References

Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Additional Details:

No block schedules for any reason.

1 weekend per month, will have time off during week to accommodate weekend schedule

This candidate will need to manage a caseload of 22-28 clients and will be expected to do 6-7 units per day (which will be a mixture of routine visits, admissions, recertifications and discharges); We desperately need a candidate who has strong clinical/assessment skills and that is fluent in navigating different EMRs.

Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended. Vehicles should be equipped with all season or snow tires during the months of November – April.
