Requirements

Experience

24 months of Hospice experience is required from any number of months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a Louisiana license or compact license

Certifications

BLS (Required)

Proof of Identification

Driver's license required

Additional Details:

Requirements/Experience: **Must have at least 12 months of Hospice experience within the past 3 years.

Schedule: ON CALL - 7 on and 7. M-Thursday 5pm-8am and Friday to Monday 5pm-8am.

On call means all visit types, prn, admissions, deaths, may have routines as well.

Territory: Ouachita, Richland, Morehouse, Union, Lincoln, Jackson, Caldwell, Franklin, West Carroll, East Carroll, Madison, Tensas, Catahoula, Winn.
