Requirements

Experience

24 months of Neurology Unit experience is required.

3 months of experience with Epic is preferred.

References

Candidates must provide 2 charge/lead references in order to be considered for this role

License

Candidates must have a Massachusetts license

Certifications

BLS (Required)

BSN required.

This position entails following a patient that has had a stroke or seizure 90 days post discharge. Follow up with the patient if they go to a SNF/LTAC, make specialist or PCP follow up appointments, medication management, home healthcare and will assist with getting the patient to a clinic if there are noted concerns.

Homecare or case management experience preferred.
