Additional Details:

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee. ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any) Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

Home Health PT - West Portland OR and Surrounding Areas (Newberg, Dundee, Willamina area)

Clinician to Patient ratios: 1/1 (*May visit up to 6 patients per day; 30 productivity points standard)

Years of Experience Required: 1 year of Skilled Home Health Experience

Weekend Required: No

On Call Required: No

Block Schedule: No

Floating Requirement: All travelers are expected to float between all facilities within a given state. travelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.

RTO Restrictions: Confirm with hiring manager

Special Requirements:

-Traveler is expected to have their own transportation.

-System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.

Attire / Scrub Color: Business casual, no jeans, scrubs allowed but not required (no specific color)

Minimum of 2 supervisory references covering 1 year (6 months for CNA’s) of employment total within the last three years within a comparable specialty/setting that they are booked
