This person will help serve the Windom HH area, including Worthington, Jackson, Luverne as well as the SNFs in that area.

Cases per day? 4-6

Will Therapist need to cover SNFs? If so, which locations? serve the Windom HH area, including Worthington, Jackson, Luverne as well as the SNFs in that area.

Will Therapist need to supervise PTAs? If so, how many? Yes- up to 2

Software used: NetSmart for HH and Net Health for SNF

Completion of an accredited physical therapy program required. Prior work experience as a therapist in a clinical setting
