Requirements

Experience

24 months of Ambulatory Clinic experience is required.

3 months of experience with Epic is preferred.

References

Candidates must provide 2 charge/lead references in order to be considered for this role

Candidates must have a Massachusetts license

Additional Details:

BSN and 2-years of experience with Neurology.

This position entails following a patient that has had a stroke or seizure 90 days post discharge. Follow up with the patient if they go to a SNF/LTAC make specialist or PCP follow up appointments medication management home healthcare and will assist with getting the patient to a clinic if there are noted concerns. homecare or case management experience preferred.

M-F 40 HPW.
