24 months of Case Management experience is required from last 12 months.

Additional Details:

A minimum of 1 year acute care experience is required to work in acute care facilities. The following are considered acute care facilities: Hospital (General Acute Care as well as Psychiatric, Specialized and Rehabilitation Hospitals, and Long Term Acute Care or LTAC), and Ambulatory Care Facilities.

Every other weekend

Weekly Guaranteed Hours: 40

Weekend Hours: 8-4:30p

Every other weekend.

Required Certifications: None ACM or CCM preferred

Additional Notes: Case Manager with experience of discharge planning in the acute hospital setting

Patient Population- geriatric, adult, peds, newborn.

Must Have Skills/Experience: Discharge planning in the acute hospital setting.

Preferred Skillset/Experience: ACM or CCM preferred. Creating discharge plans, based on insurance benefit, knowledgeable of utilization reviews, coordinating services to prevent readmission. Knowledgeable of CMS regulation and Virginia Medicaid

Common Diagnosis/Treatment: Cerebral Vascular Accident, Diabetes, Heart and Vascular, Sepsis

Required Education: Nursing Degree
