Travel Nurse (RN) - Case Management

Additional Details:

Guaranteed 40 hours per week

Mon-Fri

Must be RN Case manager with at least 2 years Acute Care Management/Discharge planning experience in the ED.

Experience must include a MINIMUM of 6 mos at a level 1/level 2 or busy trauma center ED

Prefer RN, BSN, Would consider ADN .

Case load/# patients per day/consults per day 15-20

EMR - Will use naviHealth to send referrals and plan to switch to EPIC Care Management over the next year)

Do you require InterQual criteria or Milliman experience - No, but need to have general understanding of discharge readiness

Is this position for UR, discharge planning, or both? Case Management - Discharge planning

What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) Will cover all Inpatient units throughout the hospital

Common types of patients & job duties Medical discharge (could be some social discharges)

Holiday Requirements Rotated as needed

Scrub Color/Dress Code Any color scrubs or business professional attire
