Requirements

Experience

24 months of Case Management experience is required from any number of months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a Kentucky license or compact license

Certifications

BLS (Required)

Proof of Identification

Driver's license required

Additional Details:

Hospice Experience is required

Must have at least 12 months of hospice in home settings experience within the past 3 years.

Program also services patients across the river in Illinois. Highly preferred that candidates are licensed in IL and KY.
