Requirements

Experience

24 months of role experience is required.

24 months of Case Management experience is required.

3 months of experience with Meditech is preferred.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

License

Candidates must have a Florida license or compact license

Additional Details:

Documentation system: Meditech, Midas and Careport (previously Navihealth).

· Scheduling requirements: Mon-Frii with rotating an occasional weekend.

· Patient Experience requirements: Experience in acute care hospital case management.

· Certifications: ACM or CCM helpful but not required.

Requested Skill Set: RN with bedside patient care experience; minimum 2 years hospital CM experience or MSW.
