12 months of Case Management experience is required from any number of months.

References

Candidates must provide 2 charge/lead references from any number of months in order to be considered for this role

Additional Details:

Hours: 40 hours guaranteed.

Each week the traveler will be working in Mililani for 4 days and in Kapolei for 1 Occasional home visits are required (approximately 1-3/week) so personal transportation is highly recommended.

Minimum 1 year of Primary Case/Care Management experience.

Preferred: Clinic care management experience preferred but hospital case management is acceptable. Hawaii Care/Case Management experience. Care/Case Management certification.
