Experience

24 months of role experience is required from any number of months.

24 months of Case Management experience is required from last 4 years.

Experience with Epic is required from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Mississippi license or compact license

Additional Details:

Guaranteed 40 hours per week. Mon-Fri.

Profile review required for this position

Must have at least 2 years Discharge Planning Experience in a Hospital Acute Care setting covering a variety of adult units. ER experience is a plus.

Experience must include A MINIMUM of 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge planning

Prefer RN, BSN, Would consider ADN .

Case load/# patients per day/consults per day 15-20

Do you require InterQual criteria or Milliman experience - No, but need to have general understanding of discharge readiness

Will cover all adult Inpatient units throughout the hospital

Common types of patients & job duties Medical discharge (could be some social discharges)

Holiday Requirements Rotated as needed

Scrub Color/Dress Code Any color scrubs or business professional attire

Will consult with Pharmacy as needed
