Requirements

Experience

12 months of Home Health experience is required.

Experience with Epic is required.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 36 months in order to be considered for this role

License

Candidates must have a Illinois license

BLS (Required)

Vaccinations

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Dress Code: professional looking scrubs

Required skills and minimum years of experience: Patient assessment skills, medication management, wound care, IV competency – PICC, peripheral, etc.., lab draws, point of care testing – INR, foley catheter, suprapubic catheter, ostomy care, communication, patient and family education

Shift: Monday – Friday. There may be some variation due to patient needs. On call weeknights and weekends on a rotation basis. It is work every 4th weekend, regular day shift.
