Requirements

Experience

12 months of Home Health experience is required.

Experience with Oasis is required.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 36 months in order to be considered for this role

License

Candidates must have a Illinois license

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Guaranteed Hours 40

Shift: Monday-Friday, no weekends, no call

Bed size/number of rooms for this unit/dept: 20-25 patients to case manage; must see 5-6 patients in a day

Nurse to patient ratio of this unit: 400 total census, each nurse manages 25 patients

Types of procedures required: Case manage 20-25 patients, seeing about 5-6 per day. Assessment, wound care, IV care, oasis documentation required

Additional Comments: Must be able to document in EPIC without issue

Dress Code: scrubs, business casual is okay with white smock
