24 months of Case Management experience is required from any number of months.

Candidates must have a Arizona license or compact license

Additional Details:

A minimum of 1 year acute care experience is required to work in acute care facilities. The following are considered acute care facilities: Hospital (General Acute Care as well as Psychiatric, Specialized and Rehabilitation Hospitals, and Long Term Acute Care or LTAC), and Ambulatory Care Facilities.

Guaranteed Hours: 36

Shift: Monday to Friday

Every other weekend

Patient Population: Rehabilitation patients

Must Have Skills/Experience: CM experience in an inpatient setting

Preferred Skillset/Experience: Utilization review / discharge planning experience

Common Diagnosis/Treatment: Stroke, Orthopedic, Cardiac
