Requirements
Experience
12 months of Home Health experience is required.
Experience with Homecare Homebase (HCHB) is required.
Experience with Oasis is required.
References
Candidates must provide 2 manager/supervisor references from within the past 24 months in order to be considered for this role
License
Candidates must have a license
Certifications
BLS (Required)
Vaccinations
Flu vaccination required. Religious, medical, and personal declinations accepted.

Shift: Monday to Friday
Guaranteed Hours: 40
Average 4-6 visits per day depending on type of visit for PT
Any overtime must be pre-approved.
Home Health experience required.
Facility may consider without HCHB if traveler agrees to complete online course prior to start date.
