Requirements
Experience
24 months of role experience is required.
24 months of Hospice experience is required.
Experience with Epic is preferred.
References
Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role
License
Candidates must have a Vermont license or compact license
Certifications
BLS (Required)
Vaccinations

Additional Details:

Minimum of 3 years prior community hospice experience required

Monday to Sunday; Bi-weekly Orientations on Monday

This facility requires ALL work history information (Bed count, EMR, facility type, etc.) to be complete in order to submit. Please ensure all employment gaps over 30 days are explained.

No block schedules for any reason.

Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended. Vehicles should be equipped with all season or snow tires during the months of November – April....

Nurse Patient Ratio by shift: Nurses in home setting carry caseloads of 12-14 patients. At MMRH day nurses care for 5-7 patients (all levels of care) and overnights shift up to 11

Patient Population (Diagnoses): Terminally ill (6 moth or less) -

Age Range: adults primarily-at times pediatrics in home setting Special Equipment: CADD pumps for patient-controlled analgesia, joey, feeding pumps, gomco suction pumps, portable oral suction units, BiPap, CPAP, cough assist machines, nebulizer machines, IV pumps for continuous infusions, glucose monitoring, COAGUCHECK for INR monitoring, laptops, and agency issued work phone issued
