Requirements
Experience
24 months of role experience is required.
12 months of Case Management experience is required.
3 months of experience with Meditech is required.
References
Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role
License
Candidates must have a California license
Certifications
BLS (Required)

Additional Details:

Required: Acute Care case management experience/discharge planning experience, Interqual/Milliman experience

Preferred: Familiarity with California market

Unit Procedures: Previous HCA case management experience

Case Manager responsible for arranging all post-acute needs for patients

Floating: May be floated to different inpatient units throughout the hospital but scope of work remains the same

Dress Code: Business casual or scrubs of any color

On-call / Weekend: No
