Requirements
Experience
12 months of role experience is required from last 3 years.
24 months of Case Management experience is required from any number of months.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Pennsylvania license or compact license
Certifications
BLS (Required)

Driver's license required

Additional Details:
Schedule: Monday - Friday, 8:30-5:00. 1 back up on call a week (Monday-Friday) and 1 weekend a month.
Territory: Philadelphia, Bucks, and Montgomery Counties.
Requirements/Experience: **Must have at least 12 months of Hospice experience within the past 3 years.
