Requirements
Experience
24 months of role experience is required.
24 months of Case Management experience is required.
Experience with Epic is required.
References
Candidates must provide 1 reference from within the past 36 months in order to be considered for this role
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a Virginia license or compact license
Certifications
BLS (Required)

Additional Details:
Orientation dates are: 11/18, 12/09
- 2 years inpatient discharge planning experience is required
- Must have large level 1, fast paced experience
- Must work two weekend shifts per month and one holiday per contract
- Must be prepared to float to different units
Caseload of approximately 25-30 cases per day
EMR- Epic Case Management Module
Attire: Business casual and white lab coat or royal blue scrubs
2 weekend shifts per month, 1 holiday per contract
