Patient Population (Diagnoses): Cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson’s, ostomy, catheters, MVA, etc.
Must have 3-5 years of home health exp, preferably someone who is fluent in Oasis.
This candidate will need to manage a caseload of 22-28 clients and will be expected to do 6-7 units per day (which will be a mixture of routine visits, admissions, recertifications and discharges).
Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended.
