Experience
24 months of role experience is required from any number of months.
24 months of Case Management experience is required from any number of months.
3 months of experience with Meditech is required from any number of months.
References
Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

Patient Experience requirements: Acute Care Case Management in UR,DCP and Transitions of Care -Home Health and Skilled Nursing Facility set ups.
