Experience
12 months of Home Health experience is required.
3 months of experience with McKesson is preferred.
References
Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Additional Details:
Guaranteed Hours: 36
Tuesday - Saturday. First week will be M-F to facilitate orientation
Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!
Home Health experience and OASIS experience a must as this candidate will case manage.
McKesson/Netsmart EMR experience strongly preferred.
We are looking for a candidate with no requested time off.
