Experience

12 months of role experience is required from any number of months.

24 months of Case Management experience is required from last 12 months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Additional Details:

Roles requiring state licensure only: Cannot have any hits on licensure.

Must have 2 years recent RN Case Management & discharge planning experience in a hospital setting, Experience with medical necessity/clinical reviews and denial management (i.e., Concurrent Reviews & Inter Qual), Knowledge of long-term care, Interdisciplinary Care Transitions (ICT) rounding & Knowledge of insurance requirements.

Shift will be Monday - Friday.
