Requirements

Experience

12 months of role experience is required from any number of months.

Travel experience is required from any number of months.

24 months of Home Health experience is preferred from last 12 months.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Wisconsin license or compact license

Vaccinations

Additional Information

Additional Details:

Can submit with a block scheduling request - The facility will always try and honor, but never guarantee.

Guaranteed Hours Per Week: 38 with flex to 40 hours per week

Shift requirement: 4- 10 hour days, with Wednesdays off, unless it is her weekend to work. Those weeks she will work 5- 8 hour shifts. - seeing patients in the Stevens Point and Plover area

Weekend/Holiday: Every other weekend and holiday rotation

On-call: 1-2x during the week and every other weekend.

Floating Requirements: at times, when there is a low patient census, possibility of floating to other territories to assist when needed

Charting Experience: Oasis required. Epic preferred.
