Requirements
Experience
Travel experience is required.
24 months of Case Management experience is required.
Experience with Cerner is required.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a District of Columbia license

Additional Details:

Holidays – as needed

Weekends – Every 5-6 weekends must work one shift

Start Dates: 11/25, 12/02, 12/16

Will accept RN Discharge Planner or Licensed Social Worker

Social worker must be eligible for LMSW/LGSW/LCSW - They must have taken and passed the exam already

RN or LMSW/LGSW/LCSW

Required: Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing

Work Setting skills Preferred: Home Health, Hospice, telephonic, previous charge experience

Case Management skills required: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/ Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management

Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters

Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU

Uniform: Business Casual. No scrubs

RTO - RTO must be on the submittal and cannot be changed once the profile has been sent to the manager for review.

Return Staff Policy If a nurse has held previous employment with Medstar, must be gone for a minimum of 1 year to return as a traveler.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.
