Candidates must provide 1 manager/supervisor reference in order to be considered for this role

Please note required Case Management HH and Hospice experience on resume in profile.

Requested time off during the course of this assignment MUST be noted and verified at upload.

Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience.

RN will be working in the Whitefield/Berlin, NH area.

RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly).

Netsmart/Allscripts EMR experience strongly preferred.
