Requirements

Experience

12 months of role experience is required from any number of months.

12 months of Home Health experience is required from any number of months.

Experience with Epic is required from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a Illinois license

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Guaranteed Hours 40

Shift: Monday – Friday. There may be some variation due to patient needs.

On call weeknights and weekends on a rotation basis. It is work every 4th weekend, regular day shift.

Bed size/number of rooms for this unit/dept: Home Health Care – see an average of 5 patients per day
