Requirements
Experience
12 months of Hospice experience is required.
3 months of experience with Homecare Homebase (HCHB) is required.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Connecticut license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
Must have at least 12 months of hospice in home settings experience within the past 3 years.
Saturdays and Sundays.
Guaranteed 16 up to 24 hours per week (allowing any extended documentation time needed). Mix of on call, scheduled visits, admissions, and emergent visits that arise.
Would cover both Milford and Hartford offices.
