Requirements

Experience

Travel experience is required from any number of months.

24 months of Home Health experience is preferred from last 12 months.

Experience with Oasis is required from any number of months.

Experience with Epic is required from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Michigan license

Additional Details:

Can submit with a block scheduling request - The facility will always try and honor, but never guarantee.

Guaranteed Hours: 40

Roughly 8a-430p, but depending on census there may be flexibility to start and end times.

Travel Requirements: 20-25 patients weekly, 4-5 visits per day on average. It can vary from 25-60 miles per day depending on the territory, patient census, staff availability, etc.

Mileage can be up to and over 100 miles as it is a large territory.

Active MI and WI RN license

Valid Driver's license
