Experience

12 months of Home Health experience is required from last 3 years.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a Illinois license

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Guaranteed hours: 36

Required skills and minimum years of experience: Home Health experience including OASIS documentation, EMR proficiency, IV and Wound care, Phlebotomy, Catheter care

Weekdays - will be in weekend/call/holiday rotation

Peoria Home Health call is: 1-3 days of call (after hours) it would typically be 2 shifts per month

Home Health requires autonomous practice in patient’s home, supervisors are available for support.

Dress Code: Scrubs, Professional appearance, tattoos must be covered, no facial piercings, conservative dress where no midriff is exposed, undergarments are not visible beneath shirts.
