24 months of Home Health experience is required.

Experience with Epic is required.

License

Additional Details:

Guaranteed Hours: If confirmed to 3x12-hour shifts, SSM will guarantee a min. of 36 hours/ week. If confirmed to 4x12-hour shifts, SSM will guarantee a min. of 48 hours/ week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours/week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours/ week.

North: Southern edge of Oshkosh, Eldorado, Rosendale, Van Dyne
South: Brownsville, Lomira, Mayville, Theresa, Horicon, Campbellsport, Bryon, Oakfield
East: St. Cloud, Mt. Calvary, Taycheedah, Calumet, Kiel, New Holstein
West: Ripon, Princeton, Markesan, Berlin
And everything in the middle

RTO Restrictions: Please request RTO up front if possible. Travelers may be required to participate in holiday and weekend rotation.

Travelers must be clean shaven for FIT testing.

Scrub Color: Business casual attire. If wearing scrubs, color is Teal.
