Requirements
Experience
36 months of role experience is required from any number of months.
24 months of Case Management experience is preferred from any number of months.
3 months of experience with Epic is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from any number of months in order to be considered for this role
License
Candidates must have a Connecticut license
Certifications
BLS (Required)
ACLS (Required)
Vaccinations

Additional Details:

IVS with Auto Offer (3 days RTO pre-approved).

Shift: 12A-930p, 1130a-8p, 1030a-7p, or 3p-11p.

Mandatory Overtime: During Personnel’s assignment with Client, he or she is considered part of the unit and will be included as part of the rotation pool of staff (i.e., Client employees and Personnel) who are required to work additional hours on an as-needed basis to ensure coverage for the unit.

BSN required.

CCM or ACM Preferred.
